Data Submission

If your selection requires submitted values, your quote or invoice email will include the UDS Action Areas Data Submission Form. Complete the workbook and reply to that email with the finished file attached. DOMAS will review the submission and send your secure payment link after the data are received.

Do not send or upload patient-level data, PHI, EHR exports, screenshots, or chart-level files. Only the requested aggregate numerator and denominator values should be submitted.
